As part of this form, you have two options for filing your claim. If you do not have Proof of Purchase, you must choose Option A. If you have Proof of Purchase, you may choose either Option A or Option B, but not both.

  •  Option A: I purchased RITAS Brand Product(s), but I do not have Proof of Purchase.
  •  Option B: I purchased RITAS Brand Product(s), and I do have Proof of Purchase.

Note: If you have Proof of Purchase for some or all of the Products purchased from January 1, 2018, through July 19, 2022, you may submit a Claim with Proof of Purchase or a Claim without Proof of Purchase, not both.
